Transitional Care After Hospitalization is at the forefront today in health care. SILVERcare is experienced in caring for you in your home while you recover from being in the hospital. Speak to your discharge planner or case manager about SILVERcare’s services before you leave the hospital.

We offer are dusting, laundry, linen change, mopping, trash removal, plant care, dish washing, routine household and kitchen clean up.

Here at SILVERcare, we understand the difficulty, emotionally and physically,  in caring for a loved one with dementia and Alzheimer’s. Therefore, we give our caregivers Alzheimer’s and geriatric care training and participate in mandatory continuing education sessions throughout the year.

SILVERcare evaluates clients in their home environments. We will provide services and assistance through available private and community resources including assisted living centers, nursing homes, rehabilitation facilities and hospitals.

SILVERcare calls and visits clients in their homes to assess conditions, note changes and monitor the client’s safety.
